「Flesh seam trading one thing for another inside stretched skin」with「spewing penis」as fear of the unhealthy: DREAD, FRIGHT, AWE, RISK, PERHAPS IT WILL OCCUR: describes trading some money for some rental time with a service provider, trading a few brief moments of pleasure for a lifetime of illness, agony, regret, and sorrow after getting the clap or syphilis from a flashing snatch during the age of no antibiotics and only a few minutes of actual contact, and a few seconds of actual orgasm.「Strong, solid, firm」with a「sick penis」describes the probability or likelihood of a regrettable result, bad things happening PERHAPS, after only a very short time when compared to the remainder of one's life:

「If you consider risky sexual behavior a nonissue, remember this: Over one million people around the world acquire a sexually transmitted disease (STD) every day. '…We often hear the words herpes, gonorrhea, and chlamydia in a comedic context, but anyone who has an STD knows it's no laughing matter,' Superdrug explained. 'It's something you may have to live with for the rest of your life. It's scary. It's embarrassing. It's often confusing. But if you're one of those people, you're not alone. Not even close.' 」

In its original parlance, describing a thing or situation as "smells fishy" did refer to having probable cause for extreme doubt and suspicion, but most folks do not realize why innocuous fish are so maligned in this manner. A fishy or pungent-smelling discharge from the vagina or penis may signify the presence of an infection, so caution was advised...and still is today.
「Trichomoniasis (commonly referred to as 'trich') is the most common, curable sexually transmitted disease in the world. …Trichomoniasis is caused by a one-celled parasite, Trichomonas vaginalis. Trichomoniasis affects both women and men. The most common location of infection in women is the vagina, and in men it is the urethra. In women, the symptoms of trichomoniasis may include yellow-green vaginal discharge, fishy odor, pain during urination and sexual intercourse, and genital itching or irritation. Men usually do not show trichomoniasis symptoms, but some may experience discharge from the penis or burning during urination or ejaculation.」

「One in two sexually active persons will contract an STI by age 25. …Researchers estimate that at least 80% of sexually active people will have an HPV infection at some point in their lifetime. CDC data for 2013–2014 show that about 42% of men and 40% of women aged 18-59 had genital HPV at that time. …About 1 in 8 people aged 14-49 in the U.S. has genital herpes. …Symptoms of genital herpes often go unnoticed. Most people with genital herpes—close to 90%—don't know they have the infection. Globally, researchers estimate that about two-thirds of the population under age 50—more than 3.7 billion people—are infected with HSV-1. Some 140 million people aged 15-49 are infected with genital HSV-1, primarily in the Americas, Europe and Western Pacific. …In 2015, rates of the three most common reportable STIs—chlamydia, gonorrhea and syphilis—reached a record high level. The approximately 1.5 million reported cases of chlamydia represent the highest number of annual cases of any condition ever reported to CDC. …Young people ages 15 to 24 years old accounted for 65% of chlamydia diagnoses and 50% of gonorrhea diagnoses in 2015.」And so on.

Always keep in mind that these glyphs were originally created by groups of clever old men as vulgar riddles and analogies for their own entertainment (see 籒 for that) and represented Chinese society many thousands of years ago, and not as a written or spoken language. If these glyphs had been intended for language use, the sages could not have possibly designed a worse system. Is it truly possible that all these glyphs started out as off-color jokes and puzzles? Yes! It is! Sex had no shame in ancient China. Why would it? And now, there is a cover-up. No one is suggesting these glyphs depict contemporary life in China, Japan, or anywhere else. This site describes glyph meanings, not people. These glyphs however, describe many and various things, including people, long before there were any notions of becoming politically incorrect. The question to be answered is, should these glyphs continue being used, by anyone, anywhere, for expressing language? (More...) Despite how bad the human world is today, do you actually believe that the so-called good old days were any better? Or they could only have been worse?

By the way, with a limited number of the same elements used by the sages again and again to form these glyphs, you should expect to find the same explanations again and again, as tedious as that may become, and as an inherent trait of any dictionary. This site explains the dual meanings of glyphs most often ignored by other sources that provide you with only the sanctioned definition, generally.
