Most obvious elements are「rod vertical, with many times reciprocating motions」and supported underneath with the old element, 𠫔 as a「talented hand」(that is, a「rod loose and dripping stink」): Chinese sources describe the non-JIS glyph 𠫔 as the former elemental form of「talented arm with hand」(肱), and when considered with other glyphs meaning long life and longevity (寿, 菊, 彭, 壽 as examples), these elements describe the ages old practice among many species of autoeroticism of both the sexes, or as sexual activity in general, which has continued unabated for hundreds of millions of years, a LONG TIME, and to which all of us today owe our very existence. Medical professionals today still stress the importance to one's healthy existence of periodic sexual release.
　Alternatively as some insist,「vertical array of many horizontal lines」depicts the LONG terraces carved onto slopes as a method of farming to represent the mud, fecal slurry, and water standing in the paddies, built by talented hands and were「to a large extent loose dripping and stinky.」
　If the elements are「pulled out of the normal shape」and「multiple horizontal insertions with reciprocating motions as multiple bougie reamings (彐, from the opposite perspective of the medical treatment provider, i.e., that of the sufferer)」with「stinky loose dripping」the elements describe the LONG TIME necessary and multiple sessions required to treat an infected prostate and urinary tract, as well as the LONG LENGTH generated from dripping that highly viscous material.「Rod vertical」with「three, or many times rod in motion horizontal」and the「talented arm/hand」of a medical practitioner, again describing the LONG TIME required with many sessions to eliminate the LONG LENGTH of viscous material dripping from an infected rod.
