「脳が頭蓋骨から外れるような深い感覚」は、性的な考えや実際の性器、出産の苦しみなどによって引き起こされる、何らかの形の苦痛、心配、苦悩を表しています。現代では、開いた頭蓋骨に「眩惑」を伴う「忘れられない深い感覚」のように見えるものに簡略化され、「邪悪」は「悪い考え」、または「頭が空っぽで、脳がない」思考は怒り、憎しみとして表されます。一部の情報源によると、左側の構成要素は以前は「女性」でした。しかし、今では「深い感情」に置き換えられています。上記の「歪んだ、気が狂った、ゆるんだ」の右側の構成要素は「頭蓋骨に脳がいっぱい」ですが、日本語では上部が欠けていて、思考や脳などのゆるんだ物質が飛び出している頭蓋骨として表示されます。
以前は、おそらく「レイプ、痴漢、または性的捕食者による攻撃を心配する女性の脳」が、今日でも非常に現実的な脅威として描かれていたり、「女性の脳」が一般的に何かしらのことで悩んでいたり、「出産時に子宮の収縮や新生児の頭が飛び出すことに伴う苦痛を心配する女性」も描かれていたかもしれません。
