As「female with orifice meat, grubby larvae」the elements do not suggest anyone's face other than a「tonguing mouth」but as「female with a meat hole, meatus, slippery worm」in her face, the connotations strongly are of a fellatrix at work in her 'female role' (旦),「female with meat in mouth or other orifice.」
　If a woman had larvae, grubs infesting her orifice meat, then at the very least she could rely on facial beauty. No kidding though, concubines used maggots to help clean gangrenous no-longer-integrated foot infections:

　It is surmised that maggot debridement therapy was not applied facially, except in extreme cases, or, when the patient was able to dine in private during the treatment period.
